New patients, please complete ALL forms provided below, and a dedicated wellness representative will reach out to you. All documents are secured and are HIPAA compliant.
*Please take a photo of your ID as well as a “selfie” showing your face holding your ID right next to it before starting documents. The New Patient Intake form will ask you to upload these images at the very end and if you leave the document for any reason, the information you put in will be erased, complaint to HIPAA Regulations.
